Special Care Team

Our Special care team is comprised of Dr. Erasmus, Primary Care RN, Homecare RN , Acute Care head nurse and pharmacy.

The team was formed because all members have had personal experiences with dealing with difficult diagnosis’s either with themselves or a loved one; it was through these experiences that gaps were identified.

The team works together to ensure that patients who are dealing with difficult diagnoses have their care needs met in a seamless fashion.
