Risk Reduction



The Provost Primary Care Risk Reduction Program is designed to assist clients in managing health issues such as high blood pressure, high cholesterol and coronary artery disease.

Through education and goal setting the client will be given the tools and support required to assist them in making healthier lifestyle choices.

After your physician has referred you to the clinic as series of bloodwork will be drawn, and you will then have an appointment to meet one on one with a RN.

Your initial appointment will take about 1 hour, and follow-up appointments will range from 30min-1 hour.

The goal of each visit will be to set up a health plan that is realistic for you and client centered. You will be working together with health care providers to manage your health concerns through diet and lifestyle modifications.

Updates will be given to your family physician on a regular and as needed basis. Access to other members of the health care team are available as well if needed.
